`skills/code-review/SKILL.md`	第 2 組同步審查範圍。
`skills/comment-cleanup/SKILL.md`	新增註解清理技能。
`README.md`	新增技能說明與規則摘要。
`plugin.json`、`.claude-plugin/plugin.json`、`.codex-plugin/plugin.json`	同步版本。

設計重點

禁止清單只放在 references/comment-scope.md。
comment-scope.sh 負責擋，code-review 負責指出，本技能負責清理。

測試結果

sh tools/list-skills.sh 可列出 review\tcomment-cleanup。
git diff --check 通過。
ste100-lint 通過。
