需求描述：同主機不同 CLI 的版本檢查紀錄要分離，註解掃描要涵蓋審查流程痕跡。

檔案	為什麼改
`hooks/version-guard.sh`	遠端版本快取改為 `$JSC_HOME/version-cache/{CLI}/{domain}`，並相容舊路徑。
`hooks/comment-scope.sh`	新增審查流程字樣、輪次、發現編號、審查者代稱、狀態標籤偵測。
`README.md`	補狀態檔盤點與註解掃描說明。
`plugin.json`、`.claude-plugin/plugin.json`、`.codex-plugin/plugin.json`	同步版本。

設計重點

舊快取只在新路徑不存在時複製到當前 CLI。
review 樣式收斂在流程語境，避免一般技術註解誤報。

測試結果

sh -n hooks/version-guard.sh 通過。
sh -n hooks/comment-scope.sh 通過。
模擬 claude、codex、kiro 驗證快取隔離與舊路徑遷移。
臨時 repo 驗證五類新註解樣式命中。
既有 jsc 程式碼註解回歸零誤報。
git diff --check 通過。
ste100-lint 通過。
