變更摘要

補強 doc-funcs 的 README 產生規則，遇到跨專案或跨命名空間的同名型別時，功能名稱需補上可辨識的模組/專案前綴。
要求 Markdown 表格、link text、heading 中的 C# 泛型角括號先轉為 < / >，同時 anchor id 維持安全 slug。
新增 README 產生後的錨點一致性檢查，包含重複 anchor、未引用 anchor、最後一筆連結對應與最後一列 link 閉合檢查。
將 plugin manifest 版本更新為 0.0.5。
