變更摘要

• 將 code-review-resolve 工作流調整為五階段，新增執行前 git 同步流程。
• 補上 git fetch、git pull 與必要時嘗試解衝突的處理規則。
• 調整後續階段編號，將 findings 修復、分類提交、push、PR 建立順延為 B-E。
• 新增 source/base 同名時的 PR 保護流程，避免直接建立 head=base PR 或誤推目標分支。
• 將 Claude、Codex、Antigravity 三個 plugin manifest 版本 bump 至 0.1.0。
